Salary: HC8 £36,363 – £40,777 per annum (plus £5,000 market rate supplement per year)

Welcome Payment: £10,000

Contract: Permanent

Hours: 37 per week

Location: Herefordshire

Relocation Package: up to £10,000 available

Introduction

Our Children with Disabilities service is a dedicated team that includes a Team Manager, two Managing Practitioners, a Senior Practitioner, Social Workers and Family Support Workers.

We embrace a restorative approach to social work, emphasising collaboration and reflection. Senior managers provide guidance and support in complex cases.

The Role

As a Skilled Social Worker in the Children with Disabilities Team, you will work directly with children and young people who have a range of additional needs and their families. Your responsibilities include:

  • Conducting holistic assessments and creating tailored support plans
  • Coordinating services and liaising with partner agencies to deliver integrated support
  • Working collaboratively with families to promote positive outcomes and independence
  • Ensuring child‑centred decisions are made, with the child’s voice and experience at the core
  • Building trusting relationships and fostering a supportive environment for families and children
  • Maintaining up‑to‑date knowledge of legislation, best practice and protective frameworks

Qualifications

  • Passionate about supporting children and young people with disabilities and their families to achieve the best possible outcomes
  • Skilled in assessment, planning and coordinating multi‑agency support, ensuring each child’s individual needs are fully understood and met
  • Committed to working collaboratively, inclusively, and creatively with children, families and professionals from health, education and social care
  • Resilient, reflective and confident in managing complex situations and making child‑centred decisions
  • Capable of working effectively within a specialist team environment, contributing to continuous learning and improvement
  • Registered with Social Work England and hold a recognised Social Work qualification (Degree, DipSW, CQSW)
