Duration: Initial contract until 30th August 2026, Full Time

Responsibilities

• Conduct strength-based Care Act assessments to support personalised care planning
• Facilitate and support Continuing Healthcare (CHC) processes to ensure comprehensive care delivery
• Develop and review person-centred support plans, ensuring that clients’ needs and preferences are at the heart of all decisions
• Engage in safeguarding work in compliance with the Care Act 2014, protecting vulnerable adults from harm
• Collaborate within a multi-disciplinary team environment, sharing insights and strategies to enhance client outcomes
• Apply the Mental Capacity Act 2005 in practice to ensure informed and voluntary decision-making by clients

To be considered

• Registration with Social Work England
• A relevant Social Work qualification
• Experience in Adult Social Care with proven expertise in completing Care Act assessments
