Advanced Clinical Practitioner (Frailty)

We are looking for an enthusiastic Advanced Clinical Practitioner who wants to join our Urgent Community Response Team and support patients to stay at home, reducing the need for hospital admission.
The ACP will have the opportunity to work between the urgent community response service and the virtual ward.

Main duties of the job
You will work with an MDT alongside the nurse consultant, Frailty ACP, Consultant geriatrician, nurse practitioners and therapists in providing vital care in the community, completing complex assessments and minimising admission to hospital where possible.
The new Hospital at Home service enables patients to receive safe, effective treatment at home. We receive referrals from community services, NEAS, 111, GPs and Care homes, with a focus on keeping patients safe at home where possible.
We are looking for a confident communicator with strong organisational skills.
There is an expectation that the successful candidate will be instrumental in supporting and developing this evolving service, to ensure a high-quality service for our patients. With a passion for continuous development and service improvement, you will be involved in projects and trust initiatives.
Using your experience and knowledge, you will support other team members to ensure we deliver a high-quality service for our patients, providing education and support as needed.
Job responsibilities
- Provide highly specialist, person-centred care that consistently upholds patient safety, privacy, and dignity, using advanced, evidence-based clinical knowledge.
- Practice as an autonomous clinician within a multi-professional team, remaining accountable and self-directed in accordance with professional codes of conduct.
- Undertake comprehensive assessments and independently plan, deliver, and manage complete episodes of care, including complex clinical decision-making and pharmacological considerations.
- Deliver advanced clinical management for patients with urgent care needs, ensuring appropriate assessment, treatment, and care pathways in line with Trust policies, national guidelines and current legislation.
- Provide expert clinical advice, leadership, and support to ensure high-quality service delivery, demonstrating advanced clinical judgement, critical thinking, and decision-making skills.
- Act as a senior clinical leader, providing supervision, support, and performance management to junior staff and leading teams on duty to prioritise patient care effectively.
- Participate in and contribute to clinical audit, research, and quality improvement initiatives to monitor and enhance standards of care.
- Identify, report, and proactively manage clinical risks, ensuring patient safety is maintained at all times.
- Contribute to the development, implementation, and review of policies, procedures, and clinical guidelines.
- Promote evidence-based practice and integrate research findings into clinical care to ensure best practice is achieved.
- Identify and support the educational and development needs of self and colleagues, contributing to a culture of continuous learning.
- Support the planning and development of integrated services and work collaboratively with senior management and clinical leads to implement innovative ways of working.
- Operate as an autonomous practitioner within scope of practice, utilising advanced competencies including prescribing (where applicable) or Patient Group Directions (PGDs).
- Respond to and manage cases as directed by the Clinical Coordination Centre, including acting as a primary responder, co-responder, or additional clinical resource.
Qualifications & Education
- Successful completion of Advanced Clinical Practitioner Training (full Masters level equivalent)
- Successful completion of non-medical prescribing course
- Hold a recognised teaching qualification
Knowledge & Experience
- Previous experience at ACP (Foundation) Level
- Evidence of capability and commitment to teach and train other staff
- Demonstrate evidence of continuing professional development, in Emergency or Community Care
- Experience of contributing to the development of healthcare staff at a departmental / Trust level
- Evidence of teaching experience outside of a single professional group
- Significant contribution to healthcare quality improvement project(s)
- Evidence of involvement with and/or credentials in healthcare research
Skills & Abilities
- Excellent interpersonal and leadership skills
- Ability to prioritise a complex workload and work in a rapidly changing environment
