About the role

We are searching for enthusiastic people to help us support frail vulnerable adults with their individual reablement journeys to enable them to regain their independence. You will work with people discharged from hospital back into their own homes across North Northamptonshire, supporting them to achieve their goals and helping with daily living needs. No experience needed – full support and training will be provided.

Shift Pattern: 07.00 – 15.00 hrs. Every weekend. 16 hours per week.

Base: Haylock House, Kettering, NN15 6EY.

Full driving licence and access to a vehicle is essential.

What will you be doing?

  • Be genuinely concerned about people’s quality of life, enthusiastic and able to think creatively.
  • Enable individuals to regain their independence.
  • Work with customers, setting and achieving goals in their own homes in the community.
  • Respond to emergency situations in the community in partnership with health services and seniors in the team.
  • Support customers with personal care, administer medication and nutritional needs.
  • Review customers’ progress regularly, monitor the effectiveness and efficiency of the support plan, and work with seniors in the office to make adjustments as necessary.
