Lead Community Nurse – 30h/wk Fixed Term

Hywel Dda University Health Board is recruiting a Band 6 SPQ Community Nurse to deliver high-quality care in the community. You will assess, plan, implement and evaluate care to help people stay independent and reduce hospital admissions.

The post is part-time (30 hours weekly) with shifts covering a 7-day service from 08:00 to 18:00. Welsh language ability is desirable and supported by language learning options.
