We are recruiting Scheme Manager to join a core team supporting people in the community who have moved on from more complex services. Supported living services provide people an opportunity to live an independent life in their own home.
Role: Scheme Manager
Location: Durham (travel required to Bishop Auckland, Stockton and Middlesbrough)
Shifts: Days, weekends
Hours: Full-time, 40 hours per week

This is an opportunity for a pro‑active Scheme Manager to join an evolving Care Company and manage our Supported Living services in Durham. You will need:
- Previous care management experience, preferably within a supported living environment
- Hold or be working towards a NVQ Level 5 in Health & Social Care or have a suitable alternative qualification
- Excellent interpersonal skills and the ability to create an inclusive and welcoming environment for all
- Evidence that you can provide inspiring, motivational, clear, and supportive leadership

Key Responsibilities
- Takes responsibility for ensuring that each person we support has person‑centred, philosophy‑driven and outcome‑focused care/goal plans that reflect assessed and personal needs. Ensure each person is given their own preferred method of communication to input into their goal/care plans and that staff take the time to engage them.
- Take responsibility for quality within the service by implementing and ensuring compliance with the company’s clinical governance and quality assurance systems, keeping all records and documentation comprehensive and reviewed regularly, including recording of internal and external audits, and ensuring regulatory compliance at all times.
- Ensure each person’s documentation is personal, that care/goal plans are reviewed monthly and that accurate notes are made to show changes or progress. Develop effective communication systems for disseminating information including changes in care provision to all staff.
- Familiarise yourself with company policies and ensure staff are aware of them, especially safeguarding of vulnerable adults. Ensure staff recognise signs of potential abuse and know how to report them promptly.
- Understand the Mental Capacity Act 2005 and how it relates to the service users being looked after. Ensure assessments and Deprivation of Liberty Safeguards are in place as needed and that staff understand the term ‘lack capacity’.
- Act as a role model to team leaders and support staff at all times. Lead and manage an empowered, engaged, motivated and highly effective care team, encouraging them to make a difference to service users’ lives. Ensure effective recruitment, induction, training, coaching and development, completing bimonthly managerial supervisions and annual appraisals to meet competencies and PCS values. Manage change and conflict within the staff team.
- Make a difference to the lives of our people by encouraging them to maintain their level of independence and choice in a homely, personalised environment. Ensure staff work together with the people we support. Identify and document needs and behaviours, gathering information from each service user and all who know them, and enable needs to be met and behaviours managed efficiently. Ensure service‑user information is readily available to all staff in a quick‑read format.
- Assist the Registered Manager to review variances to budget within your cluster each month and take action to rectify any underperformance.
- Ensure staff rotas are cost‑effective and appropriate for the needs of our people. Make the best use of available staffing to avoid unnecessary costs.
- Participate in the managerial weekend working rota and on‑call duties.
- Ensure all people admitted to the cluster are issued with Terms and Conditions of residency and have signed an admission agreement or it is signed on their behalf. You are responsible for ensuring a 72‑hour care plan is in place from the day of admission, and any specialised training needs for staff are met before admission.
- Communicate and ensure adherence throughout the cluster to all operational, administrative, procedural, care and professional standards. Visit every person we support regularly to develop a close and trusting relationship. Lead effective team meetings monthly using the standard agenda and information from the Registered Manager.
- Take the lead on dealing with complaints, incidents and accidents, ensuring company policies and procedures are followed, and report to the Registered Manager and Governance and Quality Manager.
- Build good working relationships with regulator, Social Services Departments and any other external regulatory body. Manage annual and other inspections.
- Take responsibility for ensuring all regulatory and statutory requirements are met, including fire, health and safety, COSHH, RIDDOR, employment and compliance with appropriate Acts. Ensure associated risk assessments are undertaken and recorded, and escalating concerns to the service manager promptly.
