Occupational Therapist – Reablement Service

Chippenham, Wiltshire | Ongoing Temporary Contract

Service Care Solutions is recruiting an Occupational Therapist to join a Reablement Service within a Local Authority in Chippenham, Wiltshire. This role supports adults in regaining independence and staying safely at home after illness, injury, or hospital discharge.

Rate of Pay: £36.00 per hour (Umbrella), £28.78 per hour (PAYE)

Key Responsibilities

• Undertake Occupational Therapy assessments within the community and reablement settings.
• Support adults to maximise independence and achieve personal goals.
• Recommend appropriate equipment, adaptations, and moving and handling solutions.
• Develop and review reablement plans to promote safe and effective discharge from hospital and prevent admissions.
• Work closely with multidisciplinary teams, carers, and community services to deliver positive outcomes.

Requirements

• Degree or Diploma in Occupational Therapy.
• HCPC Registration.
• Previous experience within Reablement, Community OT, Adult Social Care, or Intermediate Care services.
• Strong assessment and care planning skills.
• Full UK Driving Licence and access to a vehicle preferred.
