  • Working Pattern: 36 hours per week

About The Role

Richmond and Wandsworth Councils are seeking experienced, qualified post-ASYE Social Workers to join our Preparing for Adulthood Social Care Team. This is an exciting opportunity to work within a dedicated, multidisciplinary team supporting young adults as they transition into adult social care, empowering them to achieve greater independence, wellbeing, and choice.

Our team includes Service Managers, Assistant Service Managers, Senior Social Workers, Social Workers, and Social Care Assessors, all working collaboratively across both boroughs to deliver high-quality, strengths-based services.

Key Responsibilities

  • Undertake strengths-based assessments of individuals and carers, identifying needs, risks, and appropriate support.
  • Develop person-centred care and support plans that promote independence and positive outcomes.
  • Work collaboratively with young adults, families, carers, health professionals, education providers, and partner agencies.
  • Apply professional social work values and principles while promoting dignity, inclusion, and respect for diversity.
  • Carry out safeguarding enquiries and risk assessments where required.
  • Respond effectively to changing priorities, making sound professional judgements, including in emergency situations.
  • Maintain accurate and timely case records in accordance with statutory requirements and professional standards.

About You

  • Current Social Work England registration and a recognised Social Work qualification.
  • Post-ASYE experience within Adult Social Care or a relevant statutory social work setting.
  • A strong working knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Adult Safeguarding responsibilities.
  • Excellent communication, decision-making, organisational, and time management skills.
  • A proactive, positive approach with a commitment to delivering outstanding services.
