Job Description:

We are seeking an enthusiastic and experienced Diabetes Specialist Nurse to join our friendly and progressive General Practice team on a part-time, fixed-term basis. This is an exciting opportunity for a motivated clinician who is passionate about delivering high-quality diabetes care and improving health outcomes for people living with diabetes.

Working collaboratively with GPs, practice nurses, pharmacists, healthcare assistants, and the wider multidisciplinary team, you will play a key role in providing expert diabetes management, patient education, and support across our practice population.

Main duties of the job

  • Deliver specialist diabetes care for patients with Type 1 and Type 2 with an excellent knowledge around injectables.
  • Undertake comprehensive diabetes reviews, including assessment, monitoring, and treatment planning.
  • Support patients with medication optimisation, including insulin initiation and titration where appropriate.
  • Provide education and self-management support to patients and their families.
  • Monitor and manage patients in line with national and local diabetes guidelines.
  • Identify and support patients at risk of diabetes-related complications.
  • Work closely with the multidisciplinary team to ensure coordinated patient care.
  • Contribute to quality improvement initiatives, audits, and achievement of QOF targets.
  • Maintain accurate clinical records and ensure effective communication across services.

Job responsibilities

The successful candidate will:

Person Specification

Qualifications

  • Registered Nurse (RN) with current NMC registration.
  • Significant experience in diabetes management within primary care, community, or specialist services.
  • Recognised diabetes qualification or evidence of advanced diabetes training.
  • Strong understanding of current diabetes management guidelines and best practice.
  • Excellent communication, organisational, and clinical decision-making skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Experience of insulin initiation and adjustment.
  • Experience using primary care clinical systems such as EMIS or SystemOne.
  • Experience contributing to service development and clinical audit activities.
