CAMEO is an award-winning CPFT Early Intervention in psychosis service offering an age inclusive service (14-65 years) across a three-year pathway. We have an exciting opportunity for an enthusiastic, motivated, compassionate, and conscientious Activities and Wellbeing coordinator, to work alongside the excellent and supportive Early intervention service of CAMEO South, based at Chesterton Medical Centre, in Cambridge City Centre.

Main duties of the job

The post holder will support a defined group of people who will benefit from a case management approach to improve quality of life and promote independence.

The post holder will be required to use their initiative when working independently when in the community and are expected to report clinical progress. They should demonstrate an attitude which respects and values service users’ and their families/carers.

The post holder will work in collaboration with community services and teams, stakeholders and Integrated community teams to creating a coordinated care plan that demonstrates social inclusion.

  • Support a defined group of service users to promote independence.
  • Ensure that service user is supported to live the life they wish, to the best of their abilities.
  • Sustain effective working relationships across all health and social service where appropriate.
  • Support the service user using health coaching techniques and/or motivational interviewing to improve their motivation and resilience to health and wellbeing.
  • To develop therapeutic relationships with individuals ensuring service user choice.
  • Signpost the individual to services and social groups that would increase their level of social interaction.
  • Help build social networks that enable the service user to participate in their chosen activities.
  • Empower the person’s family and carers by promoting health and wellbeing strategies in order to remain independent.
  • Facilitate the reduction and/or prevention of inappropriate treatments or hospital admissions.
  • To work effectively within the wider multi-disciplinary team, giving feedback on aservice users progress.
  • Keep accurate electronic records of activity and other performance indicators.
  • Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Person specification

Education /Qualification

  • NVQ Level 2/3 in health and social care management, Full health care certificate or similar qualification, or relevant experience
  • GCSE’s Grade A – C in Maths and English or equivalent.
  • You are willing to undertake The Care certificate upon appointment.

Experience

  • Experience of working with service users who live with mental health conditions.
  • Significant experience of working in a health and social care setting.
  • Experience of working independently, unsupervised, providing high quality services.
  • Evidence of working in a multidisciplinary team.
  • Working knowledge of health and social care services.
  • Experience or knowledge of health coaching and/or motivational interviewing techniques and how they can be used to facilitate positive behaviour change.
  • Experience of using a range of self-management or recovery tools and techniques

Skills and abilities

  • Ability to lead groups in creative, active, leisure encouraging positive participation with the service users and/or family & carers.
  • Ability to engage positively with service users and their families/carers.
  • Ability to undertake all statutory and mandatory training requirements in a timely manner.
  • Able to deal with sensitive issues with tact and diplomacy.
  • Able to develop effective interpersonal relationships with colleagues/stakeholders in health and community settings
  • Able to work under pressure, managing multiple tasks
  • Demonstrate an understanding of the issues surrounding mental illness.
  • Understands the duties and responsibilities under the Data Protection Act with regards to the service user’s confidentiality.
  • Can demonstrate basic IT skills and experience of updating electronic records

Physical Requirement

  • Able to coproduce the activities the service user would find beneficial and are responsive to their needs.
  • Able to accept, support and understand the difficulties of the service user and respond in a safe, professional, and compassionate manner.
  • Understanding of the value of activities in improving health and wellbeing.

Other

  • Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.
