Responsibilities
- Proactively identify patients approaching the end of their lives.
- Ensure supportive conversations are had to enable patients and their loved ones to discuss preferences and wishes.
- Work with Health Boards existing teams and patients’ loved ones to ensure holistic assessments are undertaken to support fast track assessments and ongoing care and treatment plans.
- Complete advance care planning and clear, comprehensive treatment escalation plans.
- Collaborate with the Health Board’s discharge team, multidisciplinary, and specialist palliative care teams to ensure patients are discharged safely and timely to an alternative care setting according to preference and need.
- Work closely with Marie Curie’s existing Hospice Care at Home Service to facilitate discharges into the community.
Job Details
- Band 6 aligned with AFC, £40,559 – £48,841 pro‑rata per annum plus enhancements.
- Contract: Permanent, Part‑time, 22.5 hours per week.
- Working hours: 09:00‑17:00 Monday‑Friday.
- Location: Cardiff and the Vale Hospice and community settings.
