Overview

Deliver clinical nursing care through assessing and monitoring residents’ needs, undertaking clinical procedures and administering medication. Serve as a role model and advocate for a holistic approach to care, working collaboratively with the wider care home team including catering, housekeeping, volunteers and pastoral staff.

Lead on admissions, discharges, complex clinical care needs, medical emergencies and end‑of‑life care. Build and maintain positive relationships with all stakeholders, including GPs, pharmacists, residents, relatives/friends and colleagues.

Maintain accurate records relating to clinical care (care plans, audits, incidents, changes to health, finances, health and safety and manual handling) and use data management, processing and analysis skills to support continuous improvement.

Assume a leadership role, covering senior posts as required, and deliver coaching, guidance and mentorship to staff. Contribute to the development of policies and procedures in the discipline and plan and organise complex, inter‑linked activities and tasks through project planning.

Actively seek, respond to and act upon client feedback, building relationships internally and externally and fostering a collaborative culture for the team and service users.

Responsibilities

  • Provide nursing care to residents, including medication administration and wound management.
  • Advise on illness or injury, make referrals to multi‑disciplinary teams and prepare for GP visits.
  • Lead admissions and discharges, manage complex clinical care, medical emergencies and end‑of‑life situations.
  • Maintain accurate and up‑to‑date records: care plans, clinical audits, incident reports, health and safety logs, manual handling records.
  • Cover senior posts when required and support continuity of care.
  • Plan, organise and manage inter‑linked activities and tasks through project planning, employing problem‑solving and sophisticated technical skills.
  • Produce regular report writing, data analysis, and recommend actions for improvement.
  • Contribute to policy and procedure development, considering long‑term impacts of decisions.
  • Coach and guide team members, providing mentoring and professional development support.
  • Engage stakeholders, seeking and using client feedback to shape services.
  • Build and maintain a strong personal network internally and externally; foster a collaborative team culture.

Qualifications

  • Qualified and NMC registered nurse (adult level 1).
  • Experience working with older people, those with disability or dementia.
  • Experience in a nursing home, hospital, sheltered housing or social care setting.
  • Experience conducting clinical audits and risk assessments.
  • Experience writing and reviewing care plans.
  • Knowledge of good practice in health and safety, first aid, moving and handling and infection control.
  • Experience of keeping accurate and complete records.
  • Understanding of the ageing process and the Mental Capacity Act.
  • Experience of people management and quality assurance systems.
  • Experience reviewing and developing internal practices and policies.
  • Desirable: Completion of, or willingness to complete, Supporting Students in Practice (formerly the Facilitated Learning and Assessment in Practice course).
  • Desirable: Working or willing to work towards further professional qualifications.
