About this role
Advanced Nurse Practitioner – Primary Care with a focus on delivering high-quality care to unregistered patients in a multidisciplinary team at the Hastings Primary Care Hub. The role combines clinical leadership, autonomous practice and supports vulnerable populations including those experiencing homelessness, temporary accommodation and complex health and social care needs.
Key responsibilities
- Provide assessment, diagnosis and treatment at first contact, managing minor illness and undifferentiated presentations.
- Prescribe medicines as an Independent Prescriber, ensuring plans are evidence-based.
- Support vulnerable patients, improve access to care, and manage complex physical, psychological and social needs.
- Navigate patients to appropriate care pathways – treatment and discharge, referral, or alternative services.
- Contribute to clinical governance, audits, quality improvement, service development and continuous patient care enhancement.
- Build relationships across the organisation and with external partners to support shared learning and collaborative care.
- Help develop the Hastings Primary Care Hub, shaping services and sharing best practice.
Qualifications
- First Level Registered Nurse with current NMC registration.
- Independent Prescriber qualification.
- Completed a Physical Assessment of Adults module and a recognised Minor Illness qualification.
- At least 2 years’ experience in General Practice or Urgent Care.
- Excellent clinical assessment and decision-making skills.
- Confidence working autonomously and managing clinical risk.
- Experience with service improvement, audit and clinical governance.
- Effective communication with patients, colleagues and partner organisations.
- Committed to continuous professional development and evidence-based practice.

Employment Conditions
Employment is subject to proof of eligibility to work in the UK, completion of a satisfactory enhanced DBS with barred list disclosure, and three years of references.
