Responsibilities
- Report to The Registered Manager & Associate Director of Quality and Governance.
- Ensure that Care Plans are fully completed and compliant with company policies, procedures, protocols and CQC standards.
- Conduct thorough outcome-based initial assessments of client needs, formulate person-centred care plans that promote independence and achieve agreed outcomes, identify clients’ needs, wishes, preferences, and ensure risks are identified and risk assessed appropriately.
- Ensure clients are at the centre of the assessment process and involve significant others (family members, social workers, OTs, GPs) as required, and conduct & record risk assessments in line with Lotus Home Care policies.
- Be responsible for all new care package referrals from Local Authority, Commissioning Care Group (CCG), private referrals, or any third parties.
- Handle all care plan reviews including 12‑month and ad‑hoc assessments required within the branch, and actively monitor and record all care plans and reviews as per Lotus Home Care policy and procedures.
Skills, experiences and qualifications
- Excellent organisational, administrative and diary management skills.
- Excellent communication skills.
- Strong IT skills including Excel, Word and good typing speed.
- Proven background in delivering high‑quality customer care/services.
- Experience in writing care plans.
- Experience in detailed risk assessments.
- Ability to work independently.
- Ideally previous carer/care industry experience.
- Ideally qualified to NVQ level 2 or higher in a relevant health and social care discipline.
- Must have good IT, communication and office skills.
Hours of work
The usual hours of work will be Monday to Friday. This role requires you to work 8 hours per day, with flexibility within the operating hours to meet the needs of the business.
