NELFT NHS Foundation Trust is seeking an Occupational Therapist to join the Urgent Community Response team, delivering rapid assessments in patients’ homes across Barking & Dagenham, Havering and Redbridge. You will influence care decisions to support independence and reduce hospital admissions.
You will work autonomously within a skilled MDT, using strong clinical reasoning and excellent communication to manage frailty, falls, and urgent care needs, while maintaining patient-centred care.
