Rehabilitation Specialist – Warwickshire Hospital – Fitness – Permanent – Part Time

Up to £28,972 per annum, pro rata – depending on skills and experience.

18 hours per week.

This role is part of Nuffield Health’s Social Impact Programmes and supports people with long‑term conditions through tailored, structured exercise programmes. The Rehabilitation Specialist will work within a multi‑disciplinary team to deliver both virtual and in‑person facilitation, provide emotional wellbeing support, and promote positive behaviour change.

Key Responsibilities

  • Identify and secure suitable community locations such as community centres, sport centres or local authority gyms.
  • Conduct risk assessments and site evaluations to ensure each location is safe and appropriate.
  • Set up and maintain equipment before, during, and after sessions.
  • Recruit participants, working with sites and GPLOs, and manage cohort registration and referrals.
  • Lead rehabilitation programmes of varying lengths and models (virtual and in‑person) tailored to individuals’ abilities and physical function.
  • Facilitate groups of 8–13 participants and conduct weekly 1‑to‑1 telephone sessions to set goals and offer emotional support.
  • Deliver live‑stream classes via MS Teams and in‑person exercise classes for diverse ability levels.
  • Guide individual and group learning using the programme journal and Hub.
  • Manage discharge processes when participants complete their programmes and identify deterioration for further referral or internal escalation.
  • Ensure participants complete their support journals and receive session plans in advance.
  • Administer programme assessments and use the correct entry‑point scale to adjust progressions and regressions for all abilities.
