Overview
An exciting opportunity at Royal Free London for an autonomous heart failure specialist nurse leading nurse‑led clinics, delivering expert assessment, diagnosis, and optimisation of evidence‑based therapy. As an independent prescriber you will initiate and titrate guideline‑directed treatments, support safe community‑based management, and lead admission‑avoidance pathways and virtual ward monitoring.
Responsibilities
- Lead nurse‑led outpatient clinics, providing assessment, diagnosis and optimisation of therapy for patients with new and established heart failure.
- Act as an independent prescriber, initiating and titrating guideline‑directed therapy, including diuretics and other medications.
- Develop and enhance admission‑avoidance pathways, manage patients through the virtual ward, coordinate remote monitoring and timely intervention.
- Collaborate with multidisciplinary teams, community and primary care services to ensure seamless integrated care across the heart‑failure pathway.
- Contribute to service improvement via audit, research, pathway development and digital monitoring processes.
- Mentor junior staff, act as a role model and provide cross‑cover for the wider cardiology virtual ward.
Qualifications & Requirements
- NHS registered nurse with clinical experience in cardiology or heart failure.
- Experience in independent prescribing or readiness to undertake training.
- Strong knowledge of evidence‑based guidelines for heart failure management.
- Excellent communication, leadership, and audit/research skills.
- Ability to work flexibly across outpatient clinics, virtual ward and admission‑avoidance pathways.
