Discharge Social Worker – Middleton (2-Week Contract)

Service Care Solutions Ltd is seeking a Hospital Discharge Social Worker for the Middleton Locality on a 2-week contract. You will manage discharge planning for adults, conduct capacity assessments under MCA, and work with health colleagues to coordinate safe transitions from hospital to home.

The role requires current Social Work England registration, hospital experience in adult care, strong communication, and the ability to work flexibly across sites with some home-working arrangements.
