Join HCRG Care Group as a Healthy Family Practitioner based in Preston and play a vital role in supporting children, young people, and families across Lancashire. Working as part of the Young People & Healthy Families Service, you’ll collaborate closely with Health Visitors, School Nurses, and wider multidisciplinary teams to deliver high-quality, compassionate care. This is a rewarding community-based role where you’ll empower families, promote child development, and help improve long-term health outcomes. This is a part-time fixed-term opportunity working 30 hours per week with a planned start date of 01/09/26 to 31/08/27.
Responsibilities
In this varied and impactful position, you will:
- Deliver key elements of the Healthy Child Programme, including developmental reviews (e.g., 1-year and 2.5-year checks)
- Support children and families with targeted interventions such as behaviour management and infant feeding
Job responsibilities
Additional duties may include collaborative work with multidisciplinary teams to ensure delivery of high-quality care and support across the service area.
