Responsibilities

The post holder is an independent prescribing Clinical Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Pharmacists in General Practice. In this role they will be supported by a GP who will develop, manage, support and mentor them. They will see patients with single or multiple medical problems where medicine optimisation is required (e.g., COPD, asthma). They will review the ongoing need for each medicine, monitor needs, and support patients with their medicine usage to achieve optimal outcomes. They will make recommendations to Senior Pharmacists or GPs for medicine improvement. They will undertake clinical medication reviews with patients and produce recommendations for Nurses and/or GPs on prescribing and monitoring. They will work with care home staff to improve the safety of medicine ordering and administration.

Qualifications

  • MPharm (Master’s degree in Pharmacy)
  • Mandatory registration with the General Pharmaceutical Council
  • Qualified Independent Prescriber
