Job overview

An experienced registered adult or mental health nurse is sought to support patients with formal or informal dementia or cognitive impairment diagnoses and those presenting with delirium. The role focuses on delivering clinical expertise, education and emotional support to patients, carers and multidisciplinary team members.

Main duties of the job

  • Support patients with dementia or cognitive impairment at all stages of the disease process.
  • Support patients presenting with delirium; provide education and expertise to ward staff, family members and carers.
  • Work as an autonomous practitioner, liaising with all members of the multidisciplinary team to provide clinical expertise, emotional & psychological support and education to staff, patients and their carers.
  • Provide dedicated dementia education to the Trust.
  • Facilitate evidence‑based best practice and adapt to an evolving service.
  • Build and maintain effective relationships with external services and providers.
  • Support development and line management of dementia team members (Dementia Nurses, Dementia Nurse Associate, Dementia Technician).
  • Deputise with clinical expertise in the absence of the Head of Nursing for Mental Health and Dementia.
  • Support volunteers working with patients living with dementia/cognitive impairment.
  • Act as Practice Assessor and Practice Supervisor for students (pre‑ and post‑graduate/registration) allocated to the team for placement.
  • Provide support for students of all disciplines for SPOKE experience/learning.
  • Use expertise to support dementia team staff across the Trust.
  • Effective communication and development of therapeutic relationships to ensure support for diversity and inclusion.
  • Role model effective care/treatment interventions.
  • Advisory and educational support for patients, carers and health‑care professionals.
  • Maintain a contemporary knowledge base in dementia to select and apply the most appropriate treatment regime.
  • Assess, develop, implement and evaluate individualised plans of care involving carers and relatives.
  • Assess patients presenting with delirium; identify reversible components and recommend interventions.
  • Provide support and navigation to patients and carers to anticipate and meet needs.
  • Act in an advocacy role, identifying needs for support and legal frameworks to meet capacity needs.
  • Provide support and navigation when a safeguarding referral is required or made.
  • Ensure timely flow through the hospital via effective communication and intervention.
  • Support discharge to avoid unnecessary re‑admission and provide best practice.
  • Provide leadership and intervention for patients with complex needs within the trust environment.
  • Implement and develop the Trust Strategy for Dementia.

Professional Registration

  • Registered Adult or Mental Health Nurse.

Qualifications and Experience

  • Teaching and assessment/mentorship qualification.
  • Post‑graduate qualification in dementia.
  • Evidence of academic development at Level 7.
  • Significant post‑registration advanced nursing experience in the area of specialism – recent relevant experience at Band 6 or above.
  • Demonstrated evidence‑based practice.
  • Evidence of clinical skills and abilities relevant to the role of a nurse working with patients living with dementia and their carers.
  • Understanding of recent advances in dementia research and relevant government papers.
  • Knowledge of the Mental Capacity Act, Mental Health Act, Deprivation of Liberty Safeguards and the Safeguarding of Vulnerable Adults.
