Placement & Assessment Lead (Social Care)

Achieve together is seeking an Assessment and Placement Manager to join our community hub team. You will support the organisation’s core operations by coordinating assessments, placements and occupancy planning to ensure safe, person-centred care.

You will work with Local Authority and NHS brokerage teams, referrers and families to match individuals to suitable homes, manage referrals from enquiry to admission, and help achieve occupancy and revenue targets.
