Social Worker – Transitions Team (Grade 5)

We are recruiting for Birmingham City Council in Birmingham for the Social Worker – Transitions Team (Grade 5) position. The role pays £28.40 per hour.

Office base: Sparkbrook (every Tuesday). Office attendance is required every Tuesday.

Responsibilities

You will support young people moving into adult services, completing Care Act assessments, Care Planning, and working closely with families and partner agencies to ensure smooth transitions.

Qualifications

• Qualified Social Worker with SWE registration
• Experience in transitions / adult social care
• Strong assessment and care planning skills
