Role Overview

As a Healthcare Assistant within the District Nursing Team, you will visit patients in their own homes and care homes to provide clinical care, including wound care and phlebotomy. You will follow the team leader’s instructions to deliver evidence‑based, high quality, holistic nursing care as outlined in the care plan.

Key Responsibilities

  • Visit patients in their own homes and care homes to deliver clinical and nursing care.
  • Provide wound care, phlebotomy and extended skills once proficient.
  • Follow care plans and provide evidence‑based, holistic nursing care.
  • Assist with insulin administration, other injectables, catheterisation and bowel care as part of the Trust Band 3 Extended Skills programme.

Qualifications & Requirements

  • Enhanced Disclosure and Barring Service (DBS) check – the Trust will cover the cost.
  • Valid UK right to work; visa sponsorship not available.
  • Valid driving licence and access to a car for Trust business travel.
  • Good communication, interpersonal and IT skills.
  • Demonstrable written and spoken English to a standard suitable for the role.
  • Desire to work in a primary care setting and ability to work independently.

Professional Development

Successful candidates will undertake an in‑house development programme for extended skills, covering insulin administration, injectables, catheterisation and bowel care, as well as the Care Certificate.
