Sirona care & health is seeking a Registered Pharmacy Technician to join the Medicines Optimisation team in Bristol. You will help with medicines reconciliation after discharge, support home visits, and contribute to safe transfer of care between settings.
Based in inner-city East Bristol with some regional work, you will collaborate with clinicians, attend MDT meetings, and provide guidance to teams to improve patient safety and independence with medications.
