We are seeking an experienced and dedicated Senior Social Worker to join the Restormel CMHT within Cornwall Partnership NHS Foundation Trust Mental Health Service. Role responsibilities
- This is a community‑based role that may require frequent travel to various locations throughout the county; candidates must demonstrate how they will manage these travel requirements.
- Be responsible for the assessment of care needs and clinical management of a defined caseload within the CFT Mental Health Service.
- Carry continuing responsibility for the development, implementation and evaluation of programmes of care.
- Develop the interface between primary and secondary care services so that patients/carers are offered focused and co‑ordinated packages of care that are appropriate to their needs.
About you
- Significant experience in mental health services and care coordination.
- Strong communication and organisational skills, with the ability to work collaboratively across multi‑disciplinary teams.
- Extensive experience working with mental health patients.
- Knowledge of suicide awareness and prevention and the ability to support patients.

- Relevant professional registration – Social Worker.
- Evidence of post‑basic training.
- Current professional registration.
