Frailty & Care Homes GP: Specialist Care Lead

Kent Local Medical Committee is seeking a GP with Special Interest in Frailty and Care Homes for New Hayesbank Surgery on behalf of Mid Kent PCN in Ashford. The role is fixed-term for 24 months and involves delivering high-quality, proactive care to patients with moderate-to-severe frailty and acting as the clinical lead for frailty across the PCN.

The post holder will conduct CGAs, work to reduce avoidable hospital admissions, and help patients stay independent in their preferred place of care.
