We have an exciting opportunity to join our Adult Disability Focus part of the team within the Dawlish and Teignmouth Community Health and Social Care Team. The team serves the vibrant communities of Dawlish, Teignmouth and the surrounding areas, supporting adults of working age with physical disabilities, learning disabilities and lifelong conditions, including young people transitioning into adult services. We also work with older adults experiencing physical health needs, mental health challenges and dementia, alongside providing support to carers.

Responsibilities

  • Care Act assessments and support planning
  • Mental Capacity Act assessments
  • Best Interest decision-making
  • Safeguarding enquiries and interventions
  • Court of Protection applications and proceedings
  • Supporting people through complex transitions and life changes

Qualifications

  • Is committed to strengths-based, person-centred practice
  • Has excellent assessment, recording and communication skills
  • Enjoys working collaboratively within a multidisciplinary environment
  • Can manage a varied workload and work autonomously when required
  • Is passionate about achieving positive outcomes for adults and carers

Social Worker Career Pathway

We have implemented the first phase of a new Social Worker Career Pathway within our Adult Services, with vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. The roles align with the Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for local authorities to standardise professional capabilities and offers a clear career pathway.
