We are seeking an experienced and compassionate nurse to join our team. This role involves providing assessment, intervention, and support to older people who reside in care home placements. You will work collaboratively with families, carers, and multi‑agency partners to ensure timely, safe, and effective care.
Work location: Derwent Clinic on the Shotley Bridge Hospital site, covering Derwentside and Chester‑le‑Street areas of County Durham. Service hours are 09:00 – 17:00 Monday – Friday.
Responsibilities
- Provide assessment, intervention and support to older care home residents.
- Collaborate with families, carers and multi‑agency teams to plan and deliver care.
- Maintain high standards of safety, efficacy and patient experience.
- Participate in MDT meetings and contribute evidence‑based interventions.
- Engage in regular appraisal, talent management and professional development.
Qualifications
- Current registration with the Nursing & Midwifery Council (RNMH / RNLD).
- Evidence of continuing professional development.
- Experience working with mental ill health / learning disabilities.
- Ability to work person‑centred, therapeutic and multi‑disciplinary teams.
- Understanding of relevant legislation (Mental Health Act, Mental Capacity Act).
- Knowledge of safeguarding, clinical risk assessment and clinical governance.
- Confident working knowledge of clinical medicines management including injections.
- Capacity to be recognised as practice assessor/mentor or trained clinical supervisor within an agreed timescale.
