Compassionate End-of-Life Care Assistant

Airedale NHS Foundation Trust is seeking an End of Life Care Assistant to support patients in the last hours to days of life and provide bereavement support to families. You will work with ward teams to deliver holistic, individualised care plans and help develop services to improve quality for dying patients and their loved ones.

You will offer emotional and practical support to families, liaise with palliative care colleagues, and participate in audits to enhance care quality.
