Overview
Symphony Healthcare Services is recruiting a part‑time Nurse Practitioner / Advanced Nurse / Paramedic Practitioner (must be able to prescribe) to join the dedicated team at Creech Medical Centre, Creech St Michael, near Taunton.
Responsibilities
- Provide comprehensive patient care from initial history taking through assessment, diagnosis, treatment and evaluation.
- Demonstrate safe, advanced clinical decision‑making and expert diagnostic skills.
- Work collaboratively with the general practice team to meet patient needs and support delivery of policy and procedures.
- Deliver clinician‑led triage and manage minor illness, ailments and injuries autonomously.
- Compile protocols, clinical guidelines and conduct audit, research and quality improvement activities.
- Assess and manage patient risk, including long‑term and complex conditions.
- Apply knowledge of local and national health policy, clinical governance and patient group directives.
- Communicate effectively with patients, families and multi‑disciplinary teams, including difficult messages and conflict resolution.
- Manage resources, change, and prioritise workload effectively.
- Use EMIS searches and protocols for patient management.
Qualifications
- Minimum 5 years post‑registration experience.
- At least 2 years recent primary/community nursing experience.
- Non‑medical prescribing qualification and autonomous management of minor illness.
- Advanced clinical practice skills with ability to manage long‑term and complex patient needs.
- Strong clinical examination and risk assessment abilities.
- Knowledge of primary care operating environment, national health policy and clinical governance.
- Excellent communication, negotiation, conflict management, change, resource and time management skills.
- Proficient IT skills and experience with EMIS platforms.
- Understanding of the South Somerset Symphony Programme, NHS GP contract and new models of care.
- Registered nurse, paramedic or ACP with MSc or equivalent; relevant health degree; clinical supervision training.
- Evidence of continual professional development and CPD.
- GCSE or equivalent in English and Mathematics; A‑Level qualifications or equivalent.
