Job Description Community Staff Nurse – Band 5 District Nursing n Location: Hillingdon Community Services | Central and North West London NHS Foundation Trust (CNWL) n Contract Type: Full-Time / Community Nursing n Salary: NHS Band 5 n Start Date: As soon as possible n About the Role n We are recruiting Band 5 Community Staff Nurses to join the District Nursing teams within CNWL Community Services in Hillingdon . n This is an excellent opportunity for nurses looking to develop their skills within a supportive and forward-thinking community nursing service, delivering high-quality care to patients in their own homes and community settings. n CNWL is committed to supporting professional development through a robust preceptorship programme , regular supervision, and access to extensive in-house training and clinical support. n Key Responsibilities n As a Community Staff Nurse, you will work both independently and within a multidisciplinary team to deliver safe, effective, and patient-centred care within the community. n You will develop skills including: n n n Autonomous community nursing practice n n Diabetic management, blood glucose monitoring, and insulin administration n n Tissue viability and wound care, including: n n n Compression therapy n n Doppler assessments n n VAC dressings n n Pressure ulcer management n n Surgical wound care n n n Bladder and bowel care, including catheterisation and continence assessments n n Intravenous medication administration and PICC/midline care n n Palliative and end-of-life care, including symptom management and syringe drivers n n Management of specialist drains including nephrostomies and chest drains n n You will also: n n n Maintain accurate clinical documentation and patient records n n Support patients, carers, and families with education and ongoing care needs n n Work collaboratively with specialist teams and wider community services n n Contribute to safe discharge planning and hospital admission avoidance n n What We’re Looking ForEssential n n n Registered General Nurse (RGN) with current NMC registration n n Excellent written and verbal communication skills n n Ability to work effectively within multidisciplinary teams n n Strong clinical and organisational skills n n Sound IT skills and accurate record keeping n n Understanding of risk management within community settings n n Desirable n n n Evidence of relevant CPD n n Previous experience within community or district nursing n n Why Join CNWL? n n n Comprehensive Band 5 preceptorship programme n n Dedicated support from Clinical Nurse Facilitators n n Access to specialist teams including: n n n Tissue Viability n n Diabetes n n Bladder & Bowel n n Palliative Care n n n Extensive in-house learning and development opportunities n n Opportunity to gain a broad range of advanced community nursing skills n n Supportive and collaborative working environment n…
