Islington Council’s Hospital Discharge Service is seeking a Social Worker to support adults moving from hospital to safe community settings, by coordinating assessments, care packages and liaising with hospitals, community teams and families. You will work in a person-centred, strengths-based framework under the Care Act 2014 and Home First approach, ensuring timely discharges and safe outcomes.
