About the Post

We have an opportunity for a qualified Social Worker to join our Hospital Social Work Team to work in partnership with health professionals at Barnsley’s in‑patient Intermediate Care Bed Unit. The role requires a strong understanding of the Mental Capacity Act 2005, Human Rights 1998, and Care Act 2014, and experience working with people with complex health and social care needs.

Responsibilities

As a Social Worker in the Hospital Social Work team, you will:

  • Respond to situations using strengths‑based practice, working with individuals, families, carers and communities to promote independence and make informed decisions.
  • Conduct Care Act, capacity and best‑interest assessments, and construct value for money care packages.
  • Commission and arrange appropriate services in partnership with health colleagues, including collaboration in the hospital Discharge to Assess process.
  • Balance individual needs with social care needs and health care pressures within the integrated care board partnership.
  • Maintain person‑centred focus and engage those accessing services effectively.

Qualifications & Requirements

Applicants should have:

  • Recognised social work qualification and registration with Social Work England (or pending registration).
  • Experience working with adults in a hospital or health care setting.
  • Strong communication skills and flexibility, creativity, and determination.
  • Knowledge of the Care Act, Mental Capacity Act, local and national policy, equal opportunity, and anti‑oppressive practice.
