Social Worker – Flexible Hours & Community Care Impact

Herefordshire Council is seeking a permanent Social Worker in Adult Social Care to join our Hereford-based team. You will conduct Care Act assessments, develop person-centred support plans, and collaborate with partner agencies to deliver holistic care.
