Social Worker Level 2 – West Community Wellbeing Team

We have two vacancies in the Community Wellbeing Team covering our Lincoln North & Gainsborough Team and one vacancy in the Review Team.

Responsibilities

• Work closely with adults and families to provide tailored support and care planning in a strengths‑based, person‑centred way, embracing community assets and strengths.
• Champion independence, dignity and wellbeing in every interaction.
• Collaborate with health professionals, community partners and colleagues to deliver joined‑up services.
• Bring compassion, resilience and creativity to complex situations.
• Help develop our workforce through the supervision of up to four Community Care Officers.

Requirements

• Relevant Social Work qualification and experience.
• Ability to travel around the county according to service needs.
• Strong communication and teamwork skills.
