Senior Home Care Coordinator & Advocate

Carinity is seeking a Community Care Partner to support seniors to live safely and independently at home. You will coordinate client journeys, develop and monitor budgets and care plans, and reinforce a person-centred, strengths-based approach across care teams.

You will monitor wellbeing, manage referrals and reviews, mentor Lifestyle Carers, and build strong links with families and external providers while ensuring compliance with care standards and regulations.
