Youth Mental Health Crisis Navigator

Cardiff and Vale University Health Board is seeking a clinician to lead the Single Point of Access for inpatient referrals for 13–18 year olds in Greater Manchester, coordinating with CAMHS and social care to ensure safe, timely pathways.

You will work with inpatient teams and CYP with diverse mental health needs to support discharge planning and establish effective links with external services to optimise crisis care.
