Proactive Frailty OT in Primary Care

Promni Health in the United Kingdom is seeking an experienced Occupational Therapist to join a multidisciplinary primary care team. The role delivers proactive, preventative and personalised care for patients living with frailty and complex needs.

This is a 12-month fixed-term contract, with flexible full- or part-time options. You will work across PCNs in Hornchurch, Upminster, Cranham and Romford, providing OT clinics, group consultations and collaborative care planning.
