Overview
Working with a supportive multidisciplinary team, you will deliver high-quality care so that patients can recover, regain independence and return home safely. You will play a key role in a nurse-led rehabilitation service, helping people who need rehab following illness or hospital stay. This role combines hands‑on clinical care with leadership, giving you the opportunity to guide others while improving patient outcomes.
Responsibilities
- Assess, plan and deliver personalised care for patients needing rehabilitation
- Manage a caseload, making clinical decisions including referral, treatment and discharge
- Support and supervise junior staff, acting as a role model in practice
- Work closely with other professionals to provide joined‑up care across services
- Contribute to service improvement and high standards of safe, effective care
Requirements
Essential
- Registered 1st level Nurse
- NMC Registration
- Specialist postgraduate knowledge in rehabilitation or a related subject gained through formal qualification or equivalent experience
- Mentorship Qualification
Desirable
- Non‑Medical Prescribing
Knowledge
Essential
- Comprehensive knowledge of relevant legislation, guidance and local initiatives and issues in relation to unplanned care
- Working knowledge of multi‑disciplinary teams
- Evidence of Continuing Professional Development
- Use of evidence‑based practice
Desirable
- Knowledge of the legal framework surrounding medications prescribed or supplied
Experience
Essential
- Experience of multi‑disciplinary team working and working with patients with acute medical conditions
- Experience of utilising a variety of software packages such as SystmOne, E‑Mail, Excel, PowerPoint and Word
- Experience of flexible workload
- Management and the ability to meet deadlines
- Experience and competency in supervision and support of others in prioritisation of patient care (e.g. junior staff, non‑registered staff and students)
Desirable
- Experience in Intermediate care, demonstrating effective leadership and autonomy or previous experience of working in a similar post
- Experience of the initial management, stabilisation and onward referral of patients who may self‑present with more serious or life‑threatening conditions
- Previous experience of producing reports or audits
- Previous leadership and management experience within a clinical setting
