Overview

This post offers an excellent opportunity to join our growing Hospital at Home (H@H) service as a Band 6 Nurse / Paramedic, working as part of a skilled and supportive multidisciplinary team delivering hospital-level care to patients within their own homes. Responsibilities

Operating as an integral part of the Urgent Community Response (UCR) pathway, the post holder will contribute to the safe management of patients who would otherwise require an acute hospital admission, supporting both admission avoidance and early supported discharge for the borough of Hillingdon. The post holder will carry out comprehensive clinical assessments, deliver evidence-based interventions, and actively contribute to daily MDT reviews for patients admitted to the Hospital at Home pathway. You will manage a defined caseload of patients with complex and acute needs, including those with frailty, and will be expected to make confident clinical decisions, escalating appropriately to senior clinicians and the Senior Clinical Decision Team where required. In keeping with CNWL’s SCARF framework, you will demonstrate safe, accountable and reflective practice, adapting to the varied and fast-paced nature of community-based acute care to ensure patients receive timely, effective treatment in the right place at the right time.

The role requires you to develop and maintain a broad range of clinical competencies including independent assessment in the home environment, use of digital monitoring tools, point-of-care testing, cannulation, IV therapy administration, catheter care, and frailty management. You will work in close partnership with GPs, acute hospital teams, pharmacy, social care and community nursing living CNWL’s value of partnership to ensure patients experience a seamless episode of care.
