Role Overview
An exciting opportunity has arisen within the Rochdale West Community Mental Health Team for a Community Psychiatric Nurse / Social Worker or OT to provide mental health care to people of working age who have moderate to severe mental health problems across a range of presenting conditions. The role offers a strong commitment to professional development with clinical and management supervision and a personal development and training plan.
Key Responsibilities
- Assess the health and social care needs of individuals referred to the service holistically, under agreed procedures.
- Conduct specialist mental health assessments and risk assessments, and devise appropriate care plans and risk management plans.
- Provide evidence‑based therapeutic interventions appropriate to the service users assessed needs.
- Provide direct assistance, including education and advice, and emotional and psychological support to promote re‑enablement and recovery.
- Promote choice and service user involvement in care planning and risk management.
- Ensure practice aligns with social inclusion and recovery models.
- Support and collaborate with colleagues, other MDT members and outside agencies in the best interests of the service user.
- Provide written and verbal feedback to clients, carers and relevant professionals, respecting consent, confidentiality and Information Governance guidelines.
- Signpost all carers to access Carer Assessments.
- Report concerns identified under Safeguarding Adults procedures.
- Observe the legal requirements of the Mental Health Act, Mental Capacity Act, Care Act and other relevant legislation.
- Maintain accurate, contemporaneous clinical records on all service user contact, in accordance with Trust documentation standards and Information Governance guidelines.
- Complete all necessary inputting, training and documentation related to electronic record systems.
- Use and manage resources within the service efficiently and effectively to achieve the highest quality service possible.
- Maintain a diary of appointments, expense claims and all activities undertaken, and produce records/statistical returns for audit.
- Deliver practice in accordance with your relevant professional Code of Conduct and Fitness to Practice Guidance.
- Maintain professional registration with the appropriate regulatory body and a professional portfolio in accordance with registration requirements.
- Provide advice and professional expertise to other team members, the service and partner agencies.
- Work collaboratively with the Service Manager/Team Manager to develop, implement and monitor Clinical Governance and Risk Management Initiatives.
- Report accidents, untoward incidents and complaints, participate in investigations and action plans, and ensure the working environment is safe and equipment is maintained in compliance with Health and Safety Regulations.
- Actively participate in team meetings, contribute to service development and undertake professional and other training to meet Individual Performance and Development Review objectives.
- Maintain own professional standards by keeping skills, knowledge and competence current.
- Undertake any other reasonable duty appropriate to the band when requested by Senior Staff, and comply with all Trust and departmental policies, procedures and guidelines.
- Maintain good working relationships with all patients, service users, staff, contractors and members of the public.
- Promote awareness and respect for equality and diversity and avoid discriminatory behaviour.
Professional and Personal Development
Staff will attend the Trust Mandatory Training and Induction Programme, a structured My Yearly Conversations review with the manager each year, and will be supported to identify and achieve professional development objectives.
Professional Qualifications
- Registered Mental Health Nurse, Social Worker or Occupational Therapist.
- Current registration with the appropriate regulatory body (e.g. NMC, HCPC).
- Completion of post‑qualification consolidation such as Preceptorship or the Assessed and Supported Year in Employment (ASYE).
- Desirable: qualification in psychological therapeutic interventions.
- Desirable: mentorship qualification.
Experience and Skills
- Experience in a relevant area of mental health practice.
- Current driving licence or supported driver with vehicle access for travel across the Trust footprint.
- Substantial experience of relevant post‑registration practice and managing a caseload.
- Experience working in a multidisciplinary team setting and with people with complex mental health problems.
- Experience of clinical supervision of other staff.
- Experience working within a community setting.
- Good working knowledge of therapeutic assessment techniques and interventions for managing behaviour of individuals with complex mental health needs.
- Knowledge of screening and assessment tools used for individuals with severe and complex mental health problems.
- Comprehensive understanding of Safeguarding Adults procedures, the Mental Capacity Act, Mental Health Act, Care Act and Care Programme Approach.
- Ability to coordinate and delegate the work of junior staff.
