Discharge & Active Recovery Triage Coordinator

Leeds City Council is seeking an Active Recovery Triage Co-ordinator to support safe hospital discharges from multiple sites in Leeds. The role is office-based with a 4-week rota, combining liaison duties with Neighbourhood Teams, LTHT and Adults & Health to coordinate visits and referrals.

You will manage data, prepare performance reports, and maintain accurate records while ensuring compliance with CQC requirements and safeguarding guidelines. A temporary and a permanent contract are available.
