Role Overview

Band 5 Community Nurse based in Kent, providing rehabilitation and support to patients in the community. The role is part of a 7‑day service with operating hours 8:00 am to 8:00 pm. A forward‑thinking Preceptorship scheme has been awarded a Quality Mark to support newly qualified nurses.

Responsibilities

  • Provide person‑centred care to patients and their families, ensuring high service standards.
  • Carry out assessments, plan and deliver care plans, and monitor patient progress in the community.
  • Use strong communication and organisational skills to ensure positive patient experiences at every stage of their healthcare journey.
  • Collaborate with multi‑disciplinary teams and contribute to service improvement initiatives.
  • Maintain accurate clinical records and comply with NHS and HCRG policies.
  • Operate within the 8:00 am‑8:00 pm shift schedule and support a 7‑day service.

Qualifications and Requirements

  • Registered General Nurse (RGN) or qualified equivalent and licence to practise.
  • Band 5 NHS salary with Agenda for Change terms and full NHS Pension.
  • Full UK driving licence and access to a vehicle for work purposes.
  • Confidence in using IT systems; working knowledge of Microsoft Office (Outlook, Word, Excel).
