Field Care Supervisor – Community Care & Mentorship

City & County Healthcare Group is seeking a Field Care Supervisor to support delivery of high-quality care services. The role includes supervising care workers, planning rotas with Care Coordinators, and conducting on-site observations and reviews of services.

Responsibilities also involve multidisciplinary reviews and spending substantial time in the community with some admin duties in the office. Training and development are provided to progress within the role.
