Our teams across Portsmouth are looking for a Community Healthcare Support Worker to work within the multi‑disciplinary Integrated Care Team that delivers community based services to a GP Practice population. The role involves nursing tasks performed under the guidance of registered practitioners, home visiting and lone working without direct supervision. Service hours are 8am to 8pm, with one weekend per four.
Responsibilities
- Support registered practitioners in the delivery of nursing care, including monitoring, recording and communicating patient activity and health status.
- Progress patients’ treatment plans within the guidelines set by registered practitioners and contribute to reviews of individual progress.
- Perform a range of clinical nursing skills such as skin care/pressure ulcer prevention, vital signs, eye and mouth care, stoma and catheter care, venepuncture, basic wound care and simple bandaging.
- Protect vulnerable older people, identify and mitigate risks, and maintain health, safety and security for self and others.
- Record and maintain contemporaneous paper and electronic patient records in line with the Trust’s Record‑Keeping Policy.
- Support clerical administration under the direction of qualified clinicians and accurately record all clinical findings.
- Monitor equipment for safety, report repairs or incidents, and use the Trust’s clinical incident system to record any clinical incidents, accidents or observed crimes.
- Participate in mandatory and essential training, interdisciplinary meetings, induction and ongoing development of knowledge and skills.
- Make autonomous clinical decisions in planning and prioritising patient care and referrals to other agencies as required.
- Build and maintain effective relationships with all stakeholders; manage own caseload without any budgetary responsibility.
Qualifications & Requirements
- NVQ Level3 in a health‑care related subject, or willingness to undertake NVQIII training where required.
- Full UK (or EU) driving licence and access to a vehicle for work purposes.
- Right to work in the UK at the time of application; no Skilled Worker visa sponsorship available for this post.
- Ability to communicate complex and sensitive information clearly to patients, carers and staff, including delivering unwelcome news.
- Strong empathy, persuasion and reassurance skills; ability to manage frequent emotional or distressing circumstances.
- Dexterity required for intramuscular injections, catheter insertion/removal, sutures removal and wound management; ability to lift or move patients frequently.
- Willingness to travel across Trust sites as required.
