HPFT is expanding its Pharmacy and Medicines Optimisation team with two new community-based clinical pharmacist posts in Hertfordshire. The roles involve delivering specialist clinical pharmacy services across community teams, conducting comprehensive medication reviews and driving medicines optimisation initiatives.
You will work within multidisciplinary groups, provide education to healthcare staff, and participate in late duty, weekend and on-call rotas to support seven-day services.
