Job Title: Social Worker (Adult Social Care)
Contract: Permanent
Location: Hereford City & Bromyard
Hours: 37 hours
Salary: £36,363 to £40,777 + £3,000 welcome payment (see guidance)

Key Responsibilities
- Conduct Care Act assessments and develop person‑centred support plans.
- Carry out Mental Capacity assessments and Section 42 enquiries.
- Work with diverse adult populations including those with physical, learning disabilities, mental health needs and older adults.
- Apply a strength‑based approach to empower individuals and champion independence.
- Collaborate with partner agencies to ensure holistic care and seamless support.
- Engage in CHC and s117 work.
- Participate in regular supervision and pursue professional development opportunities.
Qualifications and Experience
- Registered Social Worker with Social Work England and a qualifying degree.
- Current knowledge of legislation, guidance and best practice in adult social care.
- Demonstrated experience in person‑centred assessment and care planning.
- Proven ability to work collaboratively with external agencies and partners.
- Motivation to pursue further qualifications such as Practice Education, Best Interest Assessor or Approved Mental Health Professional.
- Full, clean driving licence.
