Job Title: Social Worker (Adult Social Care)

Contract: Permanent

Location: Hereford City & Bromyard

Hours: 37 hours

Salary: £36,363 to £40,777 + £3,000 welcome payment (see guidance)

Key Responsibilities

  • Conduct Care Act assessments and develop person‑centred support plans.
  • Carry out Mental Capacity assessments and Section 42 enquiries.
  • Work with diverse adult populations including those with physical, learning disabilities, mental health needs and older adults.
  • Apply a strength‑based approach to empower individuals and champion independence.
  • Collaborate with partner agencies to ensure holistic care and seamless support.
  • Engage in CHC and s117 work.
  • Participate in regular supervision and pursue professional development opportunities.

Qualifications and Experience

  • Registered Social Worker with Social Work England and a qualifying degree.
  • Current knowledge of legislation, guidance and best practice in adult social care.
  • Demonstrated experience in person‑centred assessment and care planning.
  • Proven ability to work collaboratively with external agencies and partners.
  • Motivation to pursue further qualifications such as Practice Education, Best Interest Assessor or Approved Mental Health Professional.
  • Full, clean driving licence.
