Integrated Support Worker – South Falls Team
We have an exciting opportunity for an Integrated Support Worker to join the South Falls Team, Midlands Partnership NHS Foundation Trust, in Staffordshire and Stoke on Trent. The successful applicant will work alongside the Operational Lead, Clinical Lead and clinical colleagues to deliver high‑quality assessment to patients across Staffordshire who present with falls.
Key Responsibilities
- Provide clinical and administrative support to the South Falls Team following agreed pathways to assist in the implementation of rehabilitation and therapy programmes.
- Review and manage equipment provision within patients’ homes.
- Work with dynamic clinicians across the Staffordshire area.
- Assist in the provision of ADL equipment and mobility aids, and assistive technology.
- Perform general administrative duties related to patient care and equipment provision.
- Understand medical conditions and the disease process to support patient care.
Qualifications and Skills
- NVQ level II/III or equivalent in Health and/or Social Care.
- Willingness to undertake courses relevant to post‑commitment to ongoing development and learning.
- Considerable recent practice with the NHS, Social Care, private or voluntary sector.
- Experience working with patients with nursing, therapy or social care needs.
- Experience working within an MDT setting.
- Experience working with care and treatment plans, reviewing and progressing as required.
