Discharge & Community Care Assistant

East Renfrewshire Council is seeking an enthusiastic practitioner to join our Intensive Services team, delivering person-centred care and enabling individuals to remain safe and supported in their own homes within East Renfrewshire.

You will contribute to assessment and review of service provision, support referral pathways for access to care at home, and arrange suitable care services with practical advice and support to promote independence. PVG check applies.
